Web3Forms dashboard submissions are set to a seven-day retention period; delivered email copies may remain with the recipient's email provider.

Clinical intake, payment, and private health information should be handled through Jean-Ann's secure clinical systems once connected.

Good Faith Estimate

Under the No Surprises Act, you may have the right to receive a Good Faith Estimate explaining the expected costs of non-emergency health care services. If you are uninsured or choose not to use insurance, you can ask Jean-Ann for a written estimate before you schedule care.
